Information for referrers

Unfortunately, Dr Parkin has closed his books to new referrals.

This situation will be reviewed regularly and a better idea of when books may reopen will be known by February 2019.

Filling out this referral form can assist you in providing the details needed. Alternatively, you may choose to write a referral letter.

Referrals can be faxed directly (9579 2460) or posted to Southside Health & Wellbeing, 12 The Avenue, Hurstville, NSW 2220.

You can contact Southside Health & Wellbeing by telephone (9579 2480) or by email on enquiries@southsidehealth.com.au if you have any enquiries.

Whilst every effort is made to find an appointment as early as possible, Dr Parkin does not provide emergency appointments or appointments outside of clinic times.

Information for families

Your GP or Paediatrician will need to refer so that you can claim a Medicare rebate and to provide relevant clinical information.

You may be asked for some basic information over the phone by the clinic before attending and you will be requested to complete some details on two forms, which you can download from here, Why We're Seeking A Referral and Details Form, or will be sent to you from the clinic. This will help to supplement the information provided by your referring doctor. Only once Dr Parkin has reviewed this and the letter from your doctor may an appointment be booked.

Whilst every effort is made to find an appointment as early as possible, Dr Parkin does not provide emergency appointments or appointments outside of clinic times.
